Please fill out this form completely. Incomplete forms will not be accepted. Bridesmaids dress orders are placed once every member of the party has submitted their measurements, size, and a 50% non-refundable deposit. Rushes are available at an additional cost. We will contact the bride once the dresses arrive. Dresses normally take 12 weeks to arrive. Please contact us if you have any questions.

Terms and Conditions

• If you cannot be measured by Fancy Frocks Bridal.Prom.Tuxedo, please consult a qualified seamstress or bridal sales associate.
• The customer is responsible for any measurements and "size to order" provided to Fancy Frocks Bridal.Prom.Tuxedo. Bridesmaids’ dresses and formal wear sizes are NOT the same as your retail clothing size. Do NOT order based on your retail size!
• Please consult the manufacturer's size chart before placing your order. Size charts are available at  www.FancyFrocksBridal.com/bridal-order-form.php.
• Please keep in mind that while almost any dress can be taken in several sizes by a good seamstress, many formal dresses may not have enough seam allowance to be let out, therefore it is better to order too big than too small.
• It is not unusual to require alterations on bridesmaids dresses so please plan accordingly.
• Your measurements will be kept on file. Fancy Frocks Bridal.Prom.Tuxedo is not responsible if your measurements change.
• For bridesmaids that will be pregnant when wearing their dress, it is especially important to order a large enough dress to accommodate your changing body. We also recommend ordering an extra yard of fabric and lining.
• If you are losing weight, order the size you wear now and alter.
• A 50% non-refundable deposit is required to order your gown.
• Special Orders must be paid for in full within 30 days of the item's arrival. After 30 days, all deposits and payments are forfeited and the merchandise will be returned to the sales floor.
• A fee of $35.00 will be charged for all returned checks.

*8% Sales Tax will be applied to all orders

*If you pay the 50% non-refundable deposit, the remaining balance will be due within 30 days of the arrival of the dress.  Special Orders must be paid for in full within 30 days of the item's arrival. After 30 days, all deposits and payments are forfeited and the merchandise will be returned to the sales floor.
